Thank you for choosing Poland Animal Hosptal to care for your pet. Please take a moment to read our Welcome Packet prior to your first appointment.
 

If you want to establish yourself as a client with our hospital, please complete our New Account Request Form. As soon as your account is created, we will send you a welcome email and ask you to call at your earliest convenience to pay the deposit and schedule your pet's appointment.
